I like cold blood + envenom with 5 combo points and 5 poison stacks... since i have 5/5 in Vile Poisons it deals HUGE amount of damage.

Dragon's breath + Polymorph + Pyroblast + (Blast wave) + (counterspell) + Blink + Fire Blast + Frost Nova + Fireball.
I am 17/44/00. I don't have PoM goodness or mad Shatter combos. Dragon's breath is used mainly as a time-staller to I can't get counterattacked while casting Polymorph.
Blast Wave and (imp) Counterspell are in parentheses due to the fact that I only use one of them in the combo. Blast Wave for Melee, Counterspell for casters.
