Rolling on the wrong gear due to over-excitement, then screaming the following in vent when they say its going to me.
"AAAAAARRGHH! IDIDNOTMENATOROLL! DON'T GIVE IT TO ME DAMMIT!"
Thankfully, this works, and it doesn't go to me, but karma seems merciful and I win my rolls every time after that
Luckily, with 3.2 you no longer have to worry about that. (:

I played Horde for two years. After a break a couple friends decided to play Alliance, so I joined them.
I tried to catch a flight from the Horde flightmaster in Tanaris.
.
I did that on my Night Elf too. Oops.
Let's see..there was the time when playing my Paladin that I bubbled the Flag Carrier in WSG. That didn't go over well. Or the time I put Blessing of Protection on the tank in Kara. Ouch.
My first experience in trying to pick up some blood of heroes @ lvl 55.
ROFL..You are not alone on this one! Ouch.

It's like a rite of passage ;)
"Blood of what? Oh, I can click it" <click> "Hey! Ou-"<whammo> "RUN! Aaaaaahhhh!" <splat>
On-topic:
Guess who didn't know the hunter talents reset with 3.2? Guess who got inv'd as dps to the new heroic 5man today? Guess who then quickly and efficiently put all their talent points together, while jousting, in time for the first boss?
...
Guess who has 'preview talent changes' on...and who forgot to press the button >.>

I was helping a priest with a quest in Nagrand. We had just finished the quest. I said my good byes, mounted up, and promptly plunged to my death.
It appears that kodos can't fly.
I lol'd

When I'm tanking and forget Righteous Fury. Happens more than you think.
I don't know what's up with you pally people, I've seen it happen a lot (reassuring though, when you realize it's only that and not some more worrying tanking problem)
I feel retarded when i mistakenly hit the cone of cold button while i'm nuking a boss at a 40 yards range.
And for the blood of heroes, yes, it's totally a rite of passage :) *oooh what's this ? /clicky*

I borrowed my friends druid to do some dailies, he wasn't able to get on that night. Anyway, I flew over Dal and got dismounted, so there's this lvl 80 Tauren Druid flying through the air, cow form with a parachute... I thought to myself, 'Damn, not going to be able to land for ages... I'll just wait till i hit Icecrown and get rid of my parachute and fall into the water."
So I sit there for like a minute waiting until...
/facedesk - I realised he had insta-flight form...
Everyone lol'd though.

I swam around the coast into STV at lvl 20. That worked out well.
I did the same thing. On a PvP realm. As two Horde players were swimming around the coast into Westfall. Yes, they LOLed.

I got my OS raid killed.
We were running OS without a mage or a druid, and while I often play alongside friends, in this case I was at home and without Vent. I play a rogue as my main, and a Mutilate rogue at that. At the time, I was extremely focused on ensuring that I was maximizing my use of all of my cooldowns, and stared obsessively at the DoTs on my target and the slow ticking down of Slice 'n Dice/Hunger for Blood.
So we get to those guys who hit you with a debuff that gives you a percentage chance to heal them. Nobody warns me about this debuff beforehand; I'd never noticed it in earlier runs because we had a Druid to get rid of it.
So of course I get the debuff, but the icon looks like a standard shadow damage DoT, so I pay no attention to it. I'm doing lovely DPS (or so I think), but I'm actually healing massive amounts to this thing that I'm fighting. The other raid members are typing in Raid chat to warn me, but my eyes are glued to buffs and debuffs, so I never see anything. To this day my friends claim that they even tried Raid Warning, but I'm sure I would have seen that.
Long story short, we wipe, and I finally look down in chat to see everybody yelling at this guy who flipped out at me (I didn't notice that either). I scroll up and cringe as I see line after line of my friends begging me to stop DPS and wait for the debuff to wear off.
They still give me flack for this, and we all have a good laugh. I still have a bad habit of ignoring raid chat during fights.

This literally happened 30 seconds ago.
I've spent about half an hour logged on looking for a group for
The Air Stands Still.
I go to guild chat and ask if anybody has it.
Apparently I got a nifty little horn to call some buddies.
...GAH.
what?
"These are formidable opponents and must be approached with caution. Use this war horn should you require aid. The death knights of Acherus will come. "
