To the...leftish, I guess northwest of Stormwind keep (little island surrounded by water) there's a portal area that will have all the cataclysm-related portals you unlock as you progress. You'll see several npcs standing around and you should see your Hyjal portal there. Beyond that though, you can just take a flight path.

Take the boat from Stormwind to Theramore. Once in Theramore, you can either fly north manually, or take a gryphon to Winterspring. (IF you are a druid, use your Moonglade teleport cooldown.)

Before you do any of this flying around stuff, simply check your logs to see if you have a "completed" quest for traveling to Hyjal. Abandon that, and you should be able to just redo it from Stormwind.

Enjoy Mount Hyjal. Hopefully, you'll be about level 82 and a bit on completing it. Then, it's off to Deepholm (you'll get a quest back in SW once you hit level 82). You can stay in Deepholm as long as you want, but I would suggest going to Uldum as soon as you hit 83. It's my favorite of the Cata zones (and I've completed it 27 times :)).
BTW: If you don't know about Cata, the questing model is to lead you from one hub to the next. Complete those quests, and get one to take you to the next place. Occasionally, you have 2 choices of which to do first, but it comes down to following a pretty fixed quest path though the Cata zones. I personally liked that very much, but others didn't.
