It's really tough but here are a few tricks.
1) Give the tank 1-2 seconds at the start of a fight to build threat before you start casting.
2) NEVER use VE. The healing adds up fast when there is a good amount of AoE damage to your group and can easily through you right over the edge on threat.
3) Only use Fade when you pull aggro. If your using it every time it's up it won't be available when you need it most. Remember fade is only a temporary dump. After 15 seconds you get all the threat back. After you fade wait 2-3 seconds before you even think of casting another spell.
4) Install Omen. It helps!
5) Get Subtly Enchant on your cloak. 2% does wonders.
6) Make sure you have Target of Target enabled so you can see if the mob is targeting you so you know when to fade.
7) If there is a Pally in the group/raid make sure you are getting Salvation over any other blessing.

To add to what TheRealAzoth said:
Make sure you have points in Shadow Affinity, which reduces threat from shadow spells.
In addition, make sure you at max range whenever possible. I know that seems obvious, but I don't know how many times I've accidentally past the melee threat mark standing too close... Staying at max range means you need 30% more threat than the tank before the mob turns on you, but in melee range you only need 10%. Although, honestly, if you are watching Omen you won't get to that point.
