Some of my classic epic fails:
Trying to be cool on the Skybreaker flying ship in Icecrown in front of a buncha DKs who were talking 'leet speek' amongst themselves (example LOL i gotz best gearz u sht sux' etc) so i tried to time my jump with my flying mount summon to get flying immediately... timed wrong... and ended up jumping down through the center of the ship (we were standing near the gnome quest npc inside the ship) through the big bomb and plummetting to my death amidst lots of whispers of LOL FAIL from the people there that saw it....
at lvl 72, thinking I could get the jump on a lvl 80 who was fishing and pvp tagged, I attacked... and was beaten to death with a fishing pole....
...danced as a human female toon....
...tried to pug heroic AN.

Just this afternoon, did a Hc AN run with a guildy and 3 random people. we somehow make it trough to the final boss without wipes, only to end up whiping 7, yes 7 times on the same boss. somehow the rogue (one of the random people) of our group manages to get him self killed within 20 secs everytime because he didnt move away from the stun (didnt listen to our suggestions either), while doing an afwull

DPS. (that is on average of the entire run)
One of the other random people was lagging like crazy and just missed the fight 4 times because of that("nets" rising inabling him to join the fight). After 7 times we give up, replace the rogue by a Spriest, and finish it without any noticable problems.
I just think its embaressing that a lvl 80 character isnt able to get at 1K DPS.
Other big fail: While lvling i repeatedly got ganked by horde several lvls higher than me, only to get them killed (I lvled with a PvP spec, decent gear, and im pretty good at dueling :) )

Maybe not epic, but recent...
Running normal AN (pug) on my 80 resto shammie, halfway to heroic geared. I was bored and the group was spamming LFG for a healer, off I went. Pally tank, frost mage, fire mage and DK. The tank insists he's run AN many times before on his main, so off we go.
First boss, no drama, all cool.
While the rest of the goup is smashing eggs for the quest (including me, having forgotten to do it on previous runs) the pally has run out the door, down the ramp to the first platform...and jumped off the side. Noone noticed till he was dead and asking for a rez. I can't get to his corpse safely, he has to release.
Second boss, all in position, know what they're doing, tank pulls. Tank goes and stands at the bottom of the web. Cue neverending mobs. The DK has had the sense to start pulling stuff back up the web to the group, telling the pally to come up. Hell, even I bolted one and windshock-bounced it to the DK after dropping earth shield on him.
Heal tank. Tank dies? Everyone has aggro from different mobs. Spam buttons...nothing happens. WTF? Group wipes. Frantically trying to work out what the hell is wrong with my toon and the pally says "I cast divine intervention so you could rez us after the wipe." Long silence from the group.
/sigh. Start rezzing...and again, the pally has died in the middle of the floor where I can't get without aggroing. This time he is less pleasant about having to release. Starts complaining he's now got to go and repair. Off he goes and we start to get bored waiting...so we agree in /s to have a pop at the boss without the pally. DK tanking, goes off beautifully, without a hitch.
Pally realises what we've done and the b****ing starts. He's an RL friend of one of the mages though, so we let him catch up (no lewt from that boss though, we made damn sure of that ;) ). He gets lost on the way to the final boss /facepalm.
Last boss, we've been faffing and waiting for this idiot for so long we just want to get it done. Make sure we know what we're doing, everyone in, tank pulls, web goes up. Tank is making no effort to do his job. Just standing there, not moving from the spikes or grabbing aggro off the elite adds, so I've had enough. Earth shield the DK, he notices he's got rocks, he tanks. Rest of the fight goes off smoothly, despite the pally being a complete waste of space.
All of a sudden, boss is at about 5% and the pally dies, from full health. My cast has been interrupted and I look to the right and yes, there it is...I've been DI'd again. Get it off quick and finish the fight. I notice I have leader (lord knows when that happened) and ask everyone not to loot. Pally reveals that he'd moved DI to his action bar by mistake and used it in his rotation.......and yes, we had been messing about long enough for it to come off cooldown. I can't face that bug pit again. Not ever.
