I am trying to get to this final quest

but I am unable to get started on this chain for some reason. People I have asked..who have done it don't remember the sequence of other chain quest that leads to the start of this one. I have deleted all other quest besides the ones coming from icecrown...
Please help...what is the sequence of quest i need to complete in order to start this one.
I have done skybreaker quest...I am level 78. I have I missed something..?
P.S. lehner has not appeared..neither has the person under the stairs
PSS I have also completed the "crusader pinnacle" and "its all fun and games" chain quest

That's my problem...seeing Lehner and getting the start of that chain quest. I have gone to his location and he is not there. I have read, using search on wowhead, that I need to complete other chain quest first. I even have someone helping me who completed the quest and even he cant seem to get to me going. Very frustrating.
If your wondering why ..I want the blade of the empty void for my tanking. My blazefurry has served me well but its time to upgrade. Getting this sword has now become a mission.
I know the eternally folded is out there ..but I want this sword.
