There is nothing glaringly wrong with your equipment and only minor debatables with your talents. Minor enough not to discuss them.
My guess would be your friends are jumping the gun, literally. You need time to build aggro with devastates, slams, HS, TC (for mobs). I can usually hold aggro with only a couple of devastates. The only time I really loose aggro is when the hunter, mage or warlock get itchy and start nailing the target before my aggro is established. My recommendation is tell them to wait an additional second or two and that should cure it. Remember to use Taunt when your target starts walking away.
You are a tank, you should be sacrificing AP & Str or Sta & Def. Unless you are off tanking then weapon AP & Str becomes very useful.
I would not say the Crystalblade is a significant detriment compared to Grom'tor's Charge though every little dps seems to help. Speeds seem to be comparable. Slight agility difference and definate armor difference but I think it speaks more to preference.
For me, intervene is iffy. The times where it seems not to work, is when by (read "your") targeted friendly has just lost the guys aggro and no one is currently attacking him. You also have to be facing your friendly target and he must be reasonably close to you. There might also be something else interferring with it though I have yet to identify what that might be.

One of the biggest and most comment mistakes i see warriors make, when their TPS is really low, is i find they never, or very rarely use Heroic Strike.
Id say anytime you have more than 50 rage, you should be using Heroic Strike. If you find your rage starved, or low on rage a lot, stop using demo shouts, and only use TC for when your tanking more than 1 mob at a time. If its just 1 thing, and you need more rage, dont pop it. You'll get hit more often, and generate more rage, which will allow your threat generation to increase.
Start running heroic mechs as soon as you can for a Sun Eater, and put executioner on it for increased threat generation.
Get loot with block value on them. One of your most significant threat generation sources is from shield slams. The more block value you have, the higher your threat generation will be.
Since your in normals and heroics mostly right now, id say you should spec for the most rage effeciency you can. Something like this:
