Update: Apparently LongDD of the Chinese guild Old Boys, made up Dota 2 veterans,
created the weapon
even earlier on September 17th, three days after looting the eye.
The legendary weapon
Sulfuras, Mano de Ragnaros
has been created by Kembria, Orc Warrior of the guild <Senseless> on Firemaw-EU. Believed to be the
world
region first for Classic WoW, it is quite a major feat, as it requires not only looting the inordinately rare legendary material

from Ragnaros, but also combining it with a Blacksmithing crafted
Martillo de Sulfuron
, that in turn requires 8
Lingote de sulfuron
(plus an extra to purchase the blacksmithing pattern) which have a chance to drop one at a time from Golemagg the Incinerator. These ingots are not BoP however, allowing them to be traded freely, so it's not uncommon for large servers with several raiding guilds to sell them to those who have already obtained the Eye.

The weapon itself is a slow, high damage two-handed mace with a damage proc, making it especially strong for Warriors and Shaman. Although there is an argument that

which also drops from Ragnaros can be an equal or slightly better Warrior weapon due to its armor reducing proc, that shouldn't detract from the fact that Sulfuras is still an exceedingly strong and competitive weapon, especially when obtained this early in the Classic content cycle.
