I have NO idea why this isn't in the Shadow priest BIS list. It's just so sick, such a solid DPS boost.

This trinket is visible on the character's hip in both forms in and out of combat. It does not have a shadowform reskin when shifted.

Once you get the Tarnished Bronze Scale, you can start looting Grimy Runes.

From my experience:
Drop from any mob 55+ (I only saw them drop in dungeons personally).
Final bosses of dungeons have 100% drop chance (for me), provided you don't have a clean rune in inventory (or bank).
Once you clean a rune, grimy runes no longer drop until you turn them in at Tanaris Elsewhen. However...

You can get two dirty runes at once. If you don't clean it, they can still drop and you can still see them, you just cannot pick them up. So do this until a second drops, then clean the first, and loot the second. This will save you a couple trips, and I wish I'd have known that when I started. Once you have a cleaned rune, they stop dropping until you turn it in. In theory you could actually get more than two at once if you clear things fast enough and are lucky enough on drops, just by leaving unlooted grimy runes up, then looting and cleaning them all in one go.

I believe which rune you get is random, and each one pairs with a book.

Rune of Homonculi, from Prophecy of Desecrated Citadel, pairs with Fury of the Sunwell
Rune of Soul's Strength, from Prophecy of Seven Visitors, pairs with Dark Riders of Karazhan
Rune of Suffering, from Prophecy of Verdant Winter, pairs with Winter Queen's Forest
Rune of Surging Light, from Prophecy of The Lost Tribe, pairs with Lost Tribe of Nazmir
Rune of The Eye, from Prophecy of Awakened, pairs with Chaos The Shattering
Rune of Barrier, from Prophecy of a City Enthralled, pairs with Occupation of Suramar

Death, from Prophecy of a King's Demise, pairs with Assault on the Broken Shore

My anecdotal evidence:
40 mob kills in the world: no drop.
Scholomance full run, last boss dropped. (Rune of Empowered Renew)
BRD full real run, last boss dropped. (Rune of Healing Circle)
Thunderaan: no drop.
Scholomance full run, dropped on trash in Jandice's room, last boss didn't drop one (had a cleaned one in inventory at that point). (Rune of The Eye)
Undead Strath, dropped off last boss. (Rune of Surging Light)
Onyxia: No drop.
LBRS ten man: One dropped from Mother Smolderweb. I didn't notice. Then another dropped from the scorpions and spider trash, and I couldn't loot it (unique). It was my fifth one, so I cleaned it, and looted the extra. (Rune of Surging Light, then Rune of Dispersion).

You only need to do five to get the trinket. I did six. You don't lose the original trinket though, so you might be able to turn all of them in, if you really wanted to. If a seventh drops, I'll come back and post a comment.

I personally got my 5 Grimy Runes from the following mobs:

Skeletal Guardians in Stratholme x3
Reanimated Corpse/Unstable Corpse in Scholomance x1
Plagued Hatchlings in Scholomance x1
