Dropped from a 恐怖图腾地卜师 for me in the Thousand Needles after about 5 minutes of farming mobs. Based on sources it seems to drop from any of the Grimtotem mobs in this area.

Anecdotally, this dropped for me from the Grimtotem NPCs on my 4th kill. I climbed up from 31.3, 36.9. I had all four meditations, but I doubt that mattered.
