In preparation for playing a Priest healer in Classic WoW I spent a lot of time doing research but found that most of the good resources were spread out across different guides, BiS lists, youtube videos, etc. This diffusion of material made it challenging to compile everything needed to play a Priest in one place. This guide is my answer to that; it serves as a one-stop-shop about healing priests in PVE for Classic WoW. The information presented here is based upon my own preparation research and experimentation. If you find inaccurate information please let me know (contact information at the bottom) and I'll take a look.
Priest Racials
Racials are important but in Classic WoW you can still be effective as any race. The following are the races that can be priests and the racials that they have. I've omitted some racials that aren't relevant such as weapon expertise.

If you are playing alliance the arguments for rolling Dwarf a pretty strong since you have Fear Ward, but its not necessarily required to successfully complete content because generally your warrior tank can hit their buttons to prevent being feared. Again, any race can be effective, so you should play the race that suits your play style unless you are absolutely min/max-ing.
Leveling Stats
Before we talk about the talents and rotations for priest leveling there are a few important things to know about the stats for your character. Most notably **Spell Hit**. Spell Hit is a big factor for efficient leveling (and indeed later for Shadow DPS in PVE if you end up going that route). The reason for this is that the game functions on a 2-roll system to determine hits/damages for every one of your casts:
Spell Hit roll to determine whether your spell lands.
Spell Crit roll to determine if your spell crits.
Thus if your Spell Hit is low you'll not only be gimping your ability to *hit* the target but you're also negating any value from crit stacking because a spell that doesn't hit can't crit (obviously). That being said it's not actually possible to max out Spell Hit. There's **always a 1% chance to miss that is unavoidable**. The best you can do is get 99% of the way there. Here is what that looks like:

It's pretty unlikely that you'll be hit capped at any point during your leveling due to the available gear itemization but you should definitely take it over crit (or other secondary stats) when given the choice. And obviously since there's a huge jump in the required Spell Hit for a target +3 levels above you, you should try very hard to just kill mobs closer to your level.
More generally, while leveling your focus should be on
gaining levels not on farming specific gear
. There are several reasons for this:
You'll be out-leveling gear very quickly as you rise in levels so spending time on gear for your current level is fairly fruitless.
Gear itemization at lower levels is kind of all over the place and a lot of the time you'll just be equipping gear that has randomized stats anyway.
You should therefor not worry too much about fine tuning your stats while leveling. Just focus on the broad-strokes core priorities for stats:
Spirit (mana/health regen)
Mana per 5 (mana regen)
Intellect (bigger mana pool)
Spell Hit (see above)
Stamina (dont die)
Save the fine tuning for stats for when you get to max level.
Leveling Talents
As far as talents go, there are 2 talents early on that are absolutely mandatory to get:
Spirit Tap
(Shadow)
Makes it so when you kill a target you get incredible mana regen following the kill which lets you immediately move on to the next target. In most situations it provides similar regen to having drank a prior level mana drink while moving towards your next pull.
Wand Specialization
(Disc)
With 5/5 points here you can simply use SW: Pain and then start wanding for plenty of DPS without needing to cast many (if any) spells to conserve mana.
So you'll start out looking
like this
.
From there your goal is to eventually (at level 40-45) get
Shadowform
. You can either respec at level 40 to gain access to it (which will make you temporarily lose 5/5 wand specialization) or you can wait until level 45 (to keep 5/5 wand specialization).
Leveling Rotation
The biggest misunderstanding new people have when first looking at a Priest spellbook during leveling is that they feel they must use every available offensive spell in the spellbook. But this is a recipe for slowness. The reason this is slow is because offensive spells on a leveling priest are incredibly mana inefficient. Things like Smite, Mind Blast, Mind Flay, Holy Fire, etc are fine when you're looking to burst something down quickly but if you always incorporate them as part of your main rotation you're just going to end up running out of mana after every individual mob and be forced to sit down and drink between each pull.
Rather than using all of your offensive spells you should instead do just the following rotation (at least until you hit 40 and have access to Shadow Form; more in this later):
Power Word: Shield
(to avoid spell pushback)
Smite
(but do not go OOM)
Shadow Word: Pain
Wand
Sometimes if you're trying to burst down a target quickly you can mix in a
Smite
or two but for the most part you can stick to just wanding after you get your
Shadow Word: Pain
up.
The damage output from that rotation alone is plenty sufficient to kill very efficiently without running out of mana. Combined with the talent
Spirit Tap
(effectively replenishing your mana after every kill) you'll essentially never run out of mana and be very efficient. And because you have things like
Power Word: Shield
and
Renew
available (with that mana you conserved) you shouldn't really ever have to stop pulling.
This efficient rotation works great up until level 40. At level 40 you have some decisions to make in terms of changing talents; you can either immediately at level 40 get
Shadowform
or wait until 45 (as mentioned above). At this stage you'll start to incorporate
Mind Blast
or
Mind Flay
into the rotation as your mana consumer instead of
Smite
:
Power Word: Shield
(to avoid spell pushback)
Mind Blast
or
Mind Flay
(but do not go OOM)
Shadow Word: Pain
Wand
Mind Blast seems attractive because its a big-bang, but it is very mana inefficient so be sure not to go OOM. Similar to the pre-40 rotation listed above, you can mix in more
Mind Blast
if you're trying to burst down a target quickly.
This should easily carry you to 60 at which point you can shift focus to healing instead of killing.
Some other misc things to note while leveling:
You
must must must
keep Inner Fire up for the armor bonus. Your survivability is dramatically improved if you learn to prevent it from dropping off.
You should always keep
Power Word: Fortitude
up for the additional health.
Use
Renew
to keep your HP up. You shouldn't need to use normal healing spells beyond
Renew
if you're following this guide, unless you pull too many or otherwise hit snags. You may even elect to pre-Renew if you're about to pull an elite and you know it will eat through your
Power Word: Shield
.
You should downrank
Psychic Scream
so that you don't incur the full mana hit from it. The increased spell ranks just increase the number of targets it fears and 99% of the time you're just going to be in the 1-3 targets range. So Rank 2 (3 targets) is really all you should ever need while leveling.
If you're fighting multiple mobs you should first get
Shadow Word: Pain
ticking on all of them and then immediately cast
Psychic Scream
so that the
Weakened Soul
debuff drops sooner. While the mobs are running around you can proceed to wand down the targets one by one. This is obviously highly dependent on how tightly packed the mobs you're fighting are because you need to be careful not to fear into them and get adds.
PVE Healing Stats
For healing the following is the stat priority with explanations. There's a great deal of variability in stat choices depending on which talents you've chosen and what gear you have but this is the general idea:
+Healing
This one should be fairly self-explanatory. It makes you better at your primary job for the same mana cost spent.
Spirit
Mana efficiency is critical in Classic WoW and spirit helps your regen rate. In addition if you spec into the Spiritual Guidance talent (you definitely should!) it will directly increase your damage and healing up to 25% of your Spirit.
MP5
Again mana efficiency is critical in Classic WoW. The reason Spirit is rated higher than MP5 despite both increasing regen is because Spirit also affects your +healing via Spiritual Guidance.
Intellect
Increases your mana pool and healing crits.
Stamina / HP5
You don't want to die do you?
PVE Healing Talents
When it comes to level 60 end-game PVE healing there are a few talent choices available, depending on your gear. In general there are only 2 mandatory talents that you'll need to get regardless of your talent build variation:
Divine Spirit
- A 30 minute spirit buff. This synergizes with Spiritual Guidance. Since spirit is you bread-and-butter, this is a no-brainer.
Spiritual Guidance
- Straight increase to healing capability based on your spirit. Again this one should be a no-brainer.
Here are the various builds that are appropriate for PVE healing:
21/30/0 - Holy
This is the go-to spec for holy priests. Some of the talent choices are flexible enough to move a single point around here and there (improved renew vs holy specialization, etc) but the general idea here is to benefit from both Spiritual Guidance and Spiritual Healing.
26/25/0 - Holy for the beginner/undergeared
This is basically the same spec as above except that it sacrifices Spiritual Healing in favor of a larger mana pool via Mental Strength. This is really only necessary for very undergeared priests or beginners who are still growing accustom to classic wow mana conservation.
31/20/0 - Power Infusion
Power infusion is a great dps boost, especially if synergized with other cooldowns. You probably don't need more than 1 priest in your raid with a PI build, and you should weigh the gear levels in your raid to determine whether its appropriate.
PVE Healing Rotation/Spells
Before discussing your spell playstyle there are a number of books that you'll want to focus on obtaining because these spells are not taught by a trainer:
Prayer of Fortitude II
Prayer of Shadow Protection I
Prayer of Healing V
Renew X
Greater Heal V
Most drop off endgame bosses but because they are BoE you can also scan the AH from time to time to see if someone has posted them.
Now, in terms of healing on a priest in a PVE setting, there isn't a "rotation" like you might have with other classes. The goal is to select the appropriate heal that will replenish a health deficit without overhealing. The reason why overhealing is critical to avoid is because mana efficiency is key for successfully downing bosses.
Generally you'll spend most of your time casting single target regular heals (lesser heal, heal, greater heal) as appropriate for the mana/health. When emergencies arise or fast damage intake scenarios you'll start to mix in
Flash Heal
. You do not want to cast
Flash Heal
a lot though because its inefficient for the mana cost / healing.
You'll also make liberal usage of
Renew
to slow the rate of incoming damage so that you have breathing room to cast your spells. Its also important to not cast
Renew
on a target that already has a renew rolling (unless its about to drop off) because it wastes the mana of the cast.
On occasion you'll need to perform AoE healing to bring a large number of party players up quickly and in that situation
Prayer of Healing
is effective and efficient, but only if you're reasonably confident that all party members receiving the healing are at enough deficit to make PoH more efficient/expedient than your single target healing. This is very situational and there are some boss fights where you'll simply never cast it at all for the reasons stated above.
Beyond specific spells, to heal effectively as a priest there are 3 core pillars to follow:
Downranking
Pre-casting / cast-cancelling
Predictive healing
Downranking
Larger healing spells require more mana, so if you're constantly using big heals that overheal you won't have enough mana to finish the fight. To that end you'll need to ensure you have downranked versions of your spells readily available.
My standard heal is Heal 2 which will heal about 25% of our main tank's health pool. And then when damage gets more intense, depending on the health deficit, I'll use either Greater Heal 1 or Greater Heal 3. And thats sort of the way I select my heals: I want a heal on my bar that will heal 25% of a tank health pool, 50% of a tank health pool, and 75% of a tank health pool. That way I can shift between those 3 points along health bars based on what I'm observing (or predicting) needs to be healed up.
I also recommend downranking Flash Heal to somewhere in the 3 to 5 range (depending on gear) and pretty much never cast the max rank of Renew. In fact, even when downranking Renew I would cast it pretty sparingly because its effects have a tendency to mostly tick when the tanks health is at 100% (due to other heals going on) and so the mana expenditure vs benefit is pretty low. There are situations where it makes a lot of sense though, to smooth out incoming damage on long-term fights (like solo healing a tank for multiple minutes on boss fights), but Renew 6 or 7 is a good balance.
But at the end of the day the downranking spells you select are going to be dependent on your gear, the people you are healing, the buffs you roll with, etc. So experiment and see what works for your situation.
Pre-casting / Cast-cancelling
The other thing that you'll need to become familiar with is cast cancelling. Often times in healing there will be situations in which you are unable to react fast enough to heal the target in time. To combat this you need to continually be casting your heal, but if the target doesn't need the heal towards the end of the cast you cancel it to conserve mana.
Predictive Healing
Healing a player in response to them having taken damage is not rocket science. You look at what the health deficit is and select the most appropriate spell to replenish that health without overhealing much. But if you always heal in this reactive fashion you won't be a good healer because sometimes the damage is too fast for you to react to and land your spells in time. Pre-casting / cast-cancelling can help with this, but you also need to learn the dungeons/raid pulls and pay attention so that you understand where damage is *about* to occur so that you can do things like get a renew rolling or begin casting a large heal on the target that's about to take damage. This can't really be taught effectively so you'll just need to learn the fights.
Practice
Beyond the general advice of using downranking effectively, pre-casting/cast-cancelling to avoid overhealing, and using predictive healing... it is important to remember that some amount of overhealing and errant spell casts will be unavoidable and its important to not over-focus on it. At the end of the day a guide isn't going to be able to teach you to be an effective healer as much as practical experience will, so the best advice about PVE spell usage is to practice it for yourself. This will take time so you should set your expectations accordingly.
5 second rule
If you run low on mana, don't forget the 5 second rule, which is that if you don't cast a spell in 5 seconds your mana regen rate will increase. This is one of the reasons people say "wand for mana" (aside from pally buff considerations) because you can still be adding value while waiting on mana regen to tick.
PVE Healing Consumables
Obviously you don't need to be rolling with all of these all the time but this should provide a good list to pull from. I've omitted world buffs here since they are fairly controversial and aren't just a simple matter of clicking an item in your bags.

These are not listed in order of "best" to "worst" because it depends on what stats you already have on your other gear; you will need to decide which stats you wish to augment to be most effective given your current stats. Also not all of these enchants will be available to you depending on things like content phases, reputation, drops, etc, so I've listed multiple options per slot.
