In-game Maps vs. Lore-version maps

I know the WoW in-game map has undergone a lot of changes since Vanilla, and that's to be expected. But how much has the lore map changed over time? Example: right now, the Legion map puts the Broken Isles just off the southeast coast of Northrend. Is it safe to assume the isles have always been there post-Sundering? Same for Kul Tiras and Zandalar with their respective locations in the BfA Beta. Would ships sailing from the Eastern Kingdoms to Kalimdor, generally speaking, have needed to navigate with these landmasses in mind BEFORE Mists/Legion/BfA?
And lore-wise, how long might journeys like this have taken? I believe there would be a difference between in-game scale and "real-world" Azeroth scale (right? Like Stormwind lore-wise would be more like movie Stormwind in size than game Stormwind?), so an Eastern Kingdoms-Kalimdor journey would be days/weeks?

Yes the landmasses were always there; they just weren't major parts of the shipping lines until the expac opened them up. But if you notice, the ships circle around the Maelstrom, meaning they have a specific route in mind already. Several days to a week from Menethil to Theramore based on the comics.

Also note that the islands added later are significantly smaller in the lore, so "missing" them would be easier.

So, there might be a lot of other places out there that we are just avoiding. Like that theory that Pandaria is on the Equator of Azeroth.
