More than anything, it's incredibly useful for questing, dungeons, and trash. It will proc as long as your attack brings it below 35% which is nice. You can proc it by doing spell damage to critters, even (tested with fel flame).

I feel like this trinket is really awesome for demonology warlocks. When sub 35% comes along I start going into soulfire cannon mode and I noticed a significant increase in dps when I got this trinket.

Btw, the buff is called "Crescendo of Suffering" for those using NeedtoKnow or other buff bars.

As an Ele Shaman, it can really make a difference knowing when this procs if I'm waiting to pop my Fire Elemental or re-drop my Searing Totem.

If you are having a hard time getting a proper healing trinket, and nobody else needs this when it drops, this is actually quite serviceable. Especially for a Holy paladin, where you have to tag something every 8-9 seconds anyway for mana regen.
