When you turn in the quest, if you wait a moment you can see a cutscene between Nils and the orcs affected with "Durnholde Syndrome."

Nils Beerot says: It has now been six weeks since your release. Are you ready to have the shackles removed?
Drull says: Can't you just see me for me, doc? Why does it always have to be about Teddy?
Tog'thar says: I'm with Drull, doc. Laverne and I are happy together. Why try and separate us?
Nils Beerot says: My official diagnosis is that you are both idiots! IDIOTS I SAY!

I happened to be standing nearby when another player turned in the quest, and it seems like Drull and Tog'thar's response can change. Here is the alternate I saw, there may be more:

Drull says: NO, DAMN IT! For the hundredth time, NO!
Tog'thar: And we don't appreciate you referring to our friends as a 'ball and chain,' doc.

Edit: apparently it's not just when you turn in the quest, it just happens every so often. Also, the random comments from Drull and Tog'thar aren't linked.

Drull says: What I want to know is this: WHY DO YOU CARE IF I HAVE A BALL AND CHAIN SHACKLED TO MY ANKLE!

Drull says: You'll be free to take it off when I die.

Tog'thar says: Frankly, I'm getting a little tired of these therapy sessions. We're fine! Just let us go.
