This item is actually a drop from Al'akir in Throne of the Four Winds. It was added in patch 4.1 along with other necks and cloaks as well as a random chance at tier 11 helm and shoulder tokens. This should open up more of an incentive to keep downing him every week instead of DE'ing his loot most of the time.
