This seems to have a 100% drop rate, but will only show up if you have a jewelcrafter in the party that does not already know how to craft it.

This Dropped off a Mammoth in Storm Peaks for me...

Very rare to drop off anything other than King Ymiron though

To clarify, this can drop in Heroic mode as well. I was under the impression it was exclusive to Normal but it is not. It dropped for me today in the heroic.
