Currently, with the drop rate being extremely low on this item and very little information around it, I wanted to make this comment to provide some more information. I participated in the MDI Tournament, which gave me access to this weapon, and was able to study it a little further and provide accurate information on how it currently operates on live servers as of 9.2.0. All testing was done with a 285 ilvl Mythic Item and I cannot currently speak as to if these numbers are static or if these change based on the ilvl of the weapon.

How it works:

As others have stated below, when you use the item on a hostile target, it will cause a Soul Fragment to spawn near the target. This cannot be cleaved down with any type of aoe and must be directly targeted and killed. Focus and Tar macros will NOT work on it, but Mouseover macros DO work. I will put an example of one below. The Soul Fragment only has 31 health, so any ability can one-shot it easily. The time itself has a 4-minute CD and successfully killing the Soul Fragment will give you a random 60-second buff, allowing you to have the weapons effects up every 3 minutes. /cast Insert Spell Name

Possible Buffs:

Boon of the End - Swell with forbidden magic, gaining size, 389 Strength, and causing your melee attacks to have a high chance of dealing 4168 Shadow damage (Boon of the End) to all enemies in front of you.
Boon of Divine Command - Your attacks have a chance to deal 4723 Arcane damage (Boon of Divine Command) to nearby enemies and increase your armor by 263.
Boon of Harvested Hope - Your attacks have a chance to cause the target to bleed for 17,781 Physical damage (Boon of Harvested Hope) over 3 sec, healing you for that amount.
Boon of Looming Winter - Your attacks have a chance to deal 10026 Frost damage (Boon of Looming Winter) and absorb the next 7095 damage dealt to you.
Boon of Assured Victory - Your attacks have a high chance to apply a plague (Rotting Decay) to your enemies, dealing 10,072 Nature damage over 8 sec, stacking up to 3 times.
