Another way of getting the Ultimate Triage achievement, is to simply use bandage after a duel.
I got my heavy bandage accidentally and easily dropped in my first run on violet hold.
Dropped off a mob in Sholazar for me at 75, a very lucky find indeed :D

You need to have 400 Band-aid skills before you can see this thing and most of the time it will drop in instances with level bigger than 75. Mine dropped from Gundrak. Since the begining of the WotLK i did about 5-8 instances and saw it dropping twice so if you want it you can either do a few instances (and you should anyway) or kill random mobs in random places until the hell freeze... i take the isntances :)A good strategy is to level your band-aids until 400 so you can be eligeble to get the manual when it drops... and it will drop.
Book drops several times in any given instance sometimes. I got mine in Violet Hold on third portal. As for the 500 band-aids achievement, I don't get it, its more of a waste of bank space (25 slots). But I usually only make them to be sold, all I do is stock up and sell em all at once for a small fortune.And yet the cloths are far more rare than netherweave cloths.
Northern Cloth Scavenging i got 7 cloths in one loot =) its usefull for farming.
You may be able to see it below 400, but you can't roll on the Manual: Heavy Frostweave Bandage until you hit a skill of 400 in First Aid.Two dropped for me from the same pull in an instance (Drak'Tharon) so I got one and passed on the other.
I've had one drop in VH for me while I had <400, I rolled and won it. So there you go.
The book dropped for me in Grizzly Hills at 74 and 391 FA while grinding out quests solo. I guess I'm just lucky like that.
