Had 2 manuals drop in Violet Hold my first run there this weekend, because only 2 players (myself + tank) had 400+ First Aid. Had 1 more book drop in the very next run (ran VH back to back). I would suggest running that instance with 400+ FA to get the book.
I got this book at 74 on the 1st mobs of VH and so did most of my otehr guildies
I have to say sorry in advance as I am not trying to be rude, but, I am going to have to call B.S. on your post Vargar. Sorry, but thats just my personal opinion. Don't seem that you would get it at 375 or rather "around 375" when no one else at all could get it until 390+. I didn't get mine until I was past 390. It was 391 exactly when I got it and I had JUST raised the skill. Two kills later it dropped.It dropped for me in Drak'Tharon Keep while on my Death Knight who only had 375 First Aid (I kept sending the cloth to my priest for tailoring). I could not loot the item (we had it set to rare for rolling) as I did not meet the requirements.
Actually you can get this book @ skill 390 (can't learn it till 400) and from the berserkers around 33,80 (coords may be off I am at work right now sorry) and the area southeast of Light's Breach camp. Great drop rate too, just find a spawn point for the said mobs and sit there and wait (they spawn quickly). i have rotated my guildies that have the skill to get through there and no one has taken more then about 5 minutes to drop it.

A way the doctor title could be given is something like, : heal 20 different raid members at least 50% hp using only heavy frostweave bandages in 1 encounter, does not count in battlegrounds.That would be really reeally hard.Med school is also really really hard.Makes sense in my eyes.It seems like it could only be used in tank and spank fights that involve some kind of aoe, and your raid would have be skilled enough to do it with few heals lol.

Borrow a feature from Team Fortress 2 maybe? Like.... heal other players using the bandaid for 500k health or more to get the title "The Doc" Or just run a few laps in over 200km/h with the hog mount :P (Valentino Rossi aka, The Doctor. An awesome motorbike racer.)

Embarrassingly I did not have my First Aid skill up to par before I read this. Suffice it to say, I spent an hour and some gold and now am up to 450 churning out Heavy Frostweave bandages like nobodies business.Thanks.
I got it frm The Nexus, first booked tht dropped my mate picked up and second mob we killed looted it again so we got 2 in very short time
So I'm guessing First Aid is pretty useeless past Heavy Frostweave?

To answer the question of the achiement for "doctor" title, maybe heal x amount of people below x% of health
Good achievement to reward the Doctor title would be to complete Grand Master First Aid, Stocking up, Ultimate Triage and a new achievement where you use a Heavy Frostweave Bandage to heal a tank who is below 10% while fighting a 80 instance boss or heal a player capturing a PvP object with less than 5% health.
