Got the recipe on my first Violet Hold run... it dropped THREE times in that one run. I was 74 and pretty sure I was over 400 skill, but don't remember. And yes, the frosweave drop rate is lame. Though it seems I've had more frequent drops than anywhere else, so far (HF,DB and GH... course, save HF, not excessively humanoid populated zones... whatever).
I got it at exactly 390 first aid from a random humanoid in Icecrown. From what I hear, 390 is the magical number. The mob also needs to be level 75 or above.
My skill was around 375 and the book dropped on my first kill as I entered the ZD zone.
Dropped for me in Zul'Drak at 390 first aid. Was able to roll on the book but not use it until 400.
I have to say sorry in advance as I am not trying to be rude, but, I am going to have to call B.S. on your post Vargar. Sorry, but thats just my personal opinion. Don't seem that you would get it at 375 or rather "around 375" when no one else at all could get it until 390+. I didn't get mine until I was past 390. It was 391 exactly when I got it and I had JUST raised the skill. Two kills later it dropped.
My Heavy Frostweave Bandage bookd dropped of one of the worgs in Grizzley Hills.
after you do get Heavy FW bandage is there any point to maxing out Firstaid, with cloth prices the way they are atm i cant see why you would bother personally

Regular (and I assume heroic as well) Violet Hold is a very easy place to get the Heavy book to drop. In one run, it dropped 3 times off trash before the first boss even came out! Everyone who didn't have it already got it.

The EASYest way would to simply lose a duel. and poof, your achivement is complete.
I can confirm at only having 380 bandaging that I got this recipe off one of the trolls you kill for the first quest from Lights Breach in Zul'DrakSo no, you do not have to have 400 skill to see the drop, though of course you need it to learn and use it.
Another, very easy way to do Ultimate Triage is to bandage the loser after a duel.
A good strategy for the Ultimate Triage is to just have a Warlock use Life Tap or Hellfire until he/she is at 5% health or less, and then bandaging them.

I was doing the Wanted! quest in Lights Breach with some guildies, both had the book and learned it already, my skill level was at 398 at the time. One of them got they're book at >400 and the other got it soon after at >400 as well. It might be a little more random than in the past. it was an undead mob that dropped it.
To go along with some of their previous movie refferences they should make the first aid achievment "It's only a flesh wound!" or maybe"All wrapped up". I'm sure they got something in the works. Kinda funny how this is what the post is about and most of you are posting about the stupid book. It's a random drop end of story.
I just wanted to point out for people who don't want to have to wait until they're 74-75 to get the heavy frostweave manual, there are friendly mobs in Zul'Drak that will attack non-friendly mobs, and unlike some cases, you can get the kill if you just hit them once and let the friendlies kill them. I snuck into Zul'Drak at 71, nearly died killing a 74 troll/raptor pair, and was about to give up and level for a bit when I saw a group of 10 mobs all attacking each other. I tagged all that I could, looted about 6 bodies, and got the manual. I believe I was at 392 first aid.Edit: Wanted to add for people who want to try this, the friendly mobs were Rageclaw, in the southwest of the zone near Light's Breach / Drak'Tharon Keep. I entered through Dragonblight and kept south of the road with the raptors. Fairly easy to avoid mobs there.
