I don't think my PC is that bad, my FPS says other wise, I have shadow's on low, the water on low, sunshafts disabled, SSAO disabled, and i'm on great but i'm only getting 20 FPS. Do I need to upgrade anything or replace something.

GeForce GT 620
When purchasing hardware that will lead to acceptable framerates, your CPU and graphics card will be the most influential components. The CPU in your system is well above entry-level CPUs and should provide acceptable performance at medium and probably higher settings. I believe your graphics card (nVidia GeForce GT 620) is holding you back. The other components aside from the video card look good, too.
The 620 is a very entry-level card. It's probably a little better than integrated graphics, but I don't think it outclasses them in any way.
If a video card upgrade is not possible, then what resolution are you running at? Lowering the resolution will greatly impact the visual quality, but it will also reduce the amount of data your GPU will need to process.
