Technically, it should be possible, as the enUS client and enGB client only differ in the servers they connect to.
I know that it is possible to switch a British client to US servers and vice versa, so it
should work with the German client as well, the only hurdle could be the login servers refusing the unsupported locale.
However, note that playing on North American or Oceanic realms, your friend will encounter only English-speaking players (or at least not many that can speak German) so this could affect the social aspect of the game.
On the other hand, being forced to communicate in English could improve your friend's language skills - I can say from experience that e.g. watching shows and using international chatrooms improves your English way more than any school education.
Altenatively, consider getting an EU account (can be attached to the same Battle.net account, though I'm unsure how account-wide features translate between regions)
Either way, make sure to take advantage of Recruit-a-Friend! If you get an EU account, let your friend recruit you for the mount :)
Edit: This post describes how to play on US servers with a French (EU) client, just change/add the portal line after installing the German client :)
Don't edit the other ones, they are for client updates - your friend may not be able to download new patches immediately, as they're released about half a day later in the EU, but as much of that time is the night from Tuesday to Wednesday in Germany, that doesn't matter much ;)

I tried the following myself with an originally enGB installation on EU realms (I'm German and play on German realms, but hated the "Germanization" of the game that 3.0 brought and switched to English) and I wasn't able to create a US trial account, but it should work the same for your friend.
If it doesn't work immediately, uninstall the game and reinstall via
the German installer and repeat step 1.
Die folgenden Informationen werden ohne Gewährleistung bereitgestellt. Benutzung auf eigene Gefahr. ;)I'm using a generic "you" for the benefit of readability, but of course mean your friend.
(scroll down for a German version)
ENGLISH:Step 1: In your WoW installation, open WTF\Config.wtf in a text editor, and alter it so that it has these three lines:
SET portal "us"
SET locale "deDE"
SET installLocale "dede"
The latter two will already exist, but set to "enUS". Change those, add the first line (which I omitted, but it is essential for you)
Step 2: Start the WoW Launcher, which will already be in German, and hit Play. There is a quick setup (~10sec) where it initializes the German data files, but after that, the game opens, the menu already in German.
Step 3: Log in with your US account and enter the game with your character.
I was able to log into a character without much problems, text such as the area name at the minimap was blank for a few seconds, no longer. The game will seemlessly load required data - text is easy and fast, noticed no loading for item tooltips and quests, but voice files might take a while, so speech may not work initially.
It automatically downloads in the background, possibly in a smart way - I had no delays when clicking on NPCs.
This was with 16K DSL, however - I'm dreadly aware that broadband isn't that widespread in this country yet. There's no telling how this behaves on ISDN.
Weirdly, closing the game and opening the Launcher again does not prompt a further download, it says the game is up to date. This may change after playing for a while, though. Once that happens, just let the Launcher run while not playing.
Just in case you weren't aware, Wowhead also offers their database in
German. You can switch the language everywhere on Wowhead in the upper right. However, there aren't that many German comments here, as many German players prefer
Buffed's database.
It's best to use both - Buffed has informative comments in German, while Wowhead offers better database features and tools.
Hope this helps you/your friend getting around in WoW with hints written in the mother tongue :)
SET portal "us"
SET locale "deDE"
SET installLocale "dede"
