NOTE: Apparently, according to lore. Players never entered Naxxramas and killed the bosses
The mysterious artifact
During the assault on Blackrock Spire, Alexandros' forces kill an orc warlock who was wielding a powerful artifact. Upon touching this artifact Alexandros' arm is immediately corrupted and no manner of healing works on it. Alexandros holds onto this artifact for many years and when it seems that the world is in dire need of help he decides to meet with a group of trusted paladins and friends. He shows them the artifact but they are wary of it. One of the paladins tries to destroy it but the artifact absorbs the light.
Alexandros has the idea that if all of them focus their powers they may be able to cleanse the artifact. They all focus all their strength into cleansing the artifact and as the artifact begins to get cleansed, Alexandros arm heals. Alexandros decides that making a weapon out of this material would create the ultimate weapon against the scourge.
The creation of the Ashbringer
Alexandros knows that the worlds best craftmen are the dwarves and makes his way to Ironforge. There they meet with King Magni Bronzebeard. Believing his brother, Muradin, to be dead Magni agrees to forge a weapon unlike any the world has ever seen. He pours all his rage and hate into making the sword.
Using the Ashbringer, Alexandros slays so many undead that the blade and the wielder both become the legend simply known as the Ashbringer.
Betrayal and the Fourth Horseman
Alexandros' son Renault is convinced by the demon Balnazaar that he must kill his father if he is to become strong. Renault leads Alexandros into a trap and while Alexandros is weak he stabs him through the back with the Ashbringer. Alexandros gets risen as a Death Knight in the service of the Lich King wielding the Corrupted Ashbringer. Alexandros is then chosen to be one of the Lich King's elite four horsemen.
The "rescue" attempt
Darion, after hearing from a troll whose life was spared by his father that his fathers soul had not yet left this world believed that his father was still alive. Darion sneaks into Naxxramas along with a small party. There they meet with Anub'Rekhan, Thaddius and finally, the four horsemen. Everyone but Darion dies but Darion kills Alexandros in self defense and Alexandros' soul gets trapped within the Ashbringer.
Now wielding the Ashbringer, Darion begins to hear hi fathers voice. Alexandros guides Darion out of the fortress and through a portal which leads back to the plaguelands. Near the end of Darion's escape he notices that he is now in Northrend. The Lich King resurrect's the three of the four horsemen and any of his minions that darion's party killed (possibly along with the party members themselves).
Family Reunion
Under the guidance of his fathers soul Darion goes to visit his brother Renault who is now the leader of the Scarlet Crusade. Renault, now driven mad, claims that he has no family and attempts to kill his brother. He knocks Darion to the ground but before he can kill Darion, Alexandros spirit appears from the Ashbringer. Renault begs forgiveness from his father. Alexandros responds to this by killing Renault in a single blow of the Ashbringer and announcing to him "You are forgiven".
The assault on Light's Hope Chapel (part 1)
With only three horsemen left, the Lich King decides that a strong new soldier is needed and decided to launch an assault on Light's Hope in order to get one.
The forces at Light's Hope appear to be overwhelmed and in a final moment of despiration Darion knows what he must do. He must make "the ultimate sacrifice".
Trusting in the power of the light, he thrusts the sword through his own chest creating a powerful explosion of light. Doing this free's Darion's fathers soul but in return
Darion's soul gets corrupt and Kel'Thuzad sees this as a victory nonetheless. The minions can assault Light's Hope some other day but for now he has his fourth horseman. Darion.
The Wrath of the Lich King
The Lich King decides to begin training an army of Death Knights in the Ebon Hold with the hopes of eradicating the pests that lie below. He also hopes this army will act as a diversion and bait so he can lure out and kill Tirion Fordring. He uses his new servant, Darion, as bait in this trap.
The assault on Light's Hope Chapel (part 2)
Darion and the death knights along with a band of scourge assault Light's Hope Chapel. They are successful in luring out Tirion. The Lich King, in an attempt to finish off this powerful leader, focuses a lot of his energy into a powerful attack. This loss of power results in Darion and the Death Knights breaking free from the Lich King's control. Realizing what he has done wrong, Darion interrupts the fight by aiding Tirion and ultimately saving his life. The lich king retreats.
Backup plan
After losing his new horseman, the Lich King decides to pay a visit to Stratholme. There, he orders/resurrects Baron Rivendare to take the place of Darion. He retreats to northrend to continue building an army.
To Northrend
After returning from outlands, the adventurers learn that the Lich King has been assaulting Orgrimmar and Stormwind. With no other choice, the adventurers must head to Northrend. There they will have to battle with many of the Lich Kings minions including the ones in the now relocated fortress of Naxxramas.
The adventurers must now search far and wide to prepare for the assault on the fortress. And when they are finished there... if they survive... they must face off against the Lich King himself. (Assuming of course that no Old Gods get in the way. But what are the chances of that, Right?)
TL;DR The assaults on Naxx in vanilla never happened. The inhabitant's of Naxxramas may be more powerful because they are closer to the Lich King
All of this is just off the top of my head but if i left anything out please say so and ill edit this
edit: ty to the people who noticed my errors. i hope you enjoy the updated version

slight correction nessecary here. Darion's father, Alexandros Mograine is the "Highlord Mograind" of the 4 horsemen in 40-man Naxx. The Corrupted Ashbringer is recovered during the assault by the Argent Dawn. When players brought the Corrupted Ashbringer to the Scarlet Monastery Cathedral, a cutscene began that basically said that the corrupted Ashbringer needed to be brought to Mograin's "pure" son (Darion), who was in Outlands. The next part was never in game, but was instead detailed in the Ashbringer comic series. The corrupted Ashbringer is brought to Darion Mograine in Outland, and he sees that his father's spirit is trapped within the blade, and still cursed. To free his father's spirit, Darion plunges the Ashbringer into his own heart, transferring the curse to himself. The Lich King decides that the son can serve where his father did, and raises him as a Death Knight just as he had Alexandros. Skip ahead to Wrath of the Lich King, where Darion and the Knights of the Ebon Blade break away from the Lich King's control, and Baron Rivendare is brought in to fill his place in the four horsemen.
Ty, since i had never done 40 man naxx i never knew it was his father that was the horseman. I will update the story. It seems to make more sense following the comic than the game (assuming that the players never entered Naxxramas).

I have changed around the layout and the story slightly and hopefully included all the necessary information for people to understand the lore. I have to leave some stuff out (like fairbanks and the paladin that Alexandros knew who helped free the orc umong other things) though because if i didnt i could probably write a novel on this
:D

I like how people don't know why all the Naxx bosses are back to life and in Naxx.
They are forgetting one thing, It's the Lich King, he's the king of the scourge, undeath and
necromancy
I've seen too many people complain that for no particular reason all the Naxx bosses are back to life, All the Lich King has to do is look at the corpse and poof, your alive again.
And Thanks for this, I didn't know about Darion and the Corrupted Ashbringer part.

Originally i had a story where all the bosses died and were resurrected but from what i can see this is the version blizzard has gone with. Most of this is taken from the 4-part comic "Ashbringer"
LINK
its a very interesting read and i would highly recommend it.
